Membership Agreement
- TERM & COMMITMENT
- This agreement requires a minimum 6-month commitment. The term begins on sign up day and continues on a month-to-month basis thereafter until terminated by either party with [30] days' written notice.
- PAYMENT TERMS
- Monthly dues will be charged automatically each month via [Credit Card/ACH].
- If a payment fails, a $35 late fee may apply.
- SERVICE UTILIZATION & ROLLOVER
- Monthly service credits are issued on the payment date.
- Rollover Policy: Unused treatments roll over for a maximum of 30 days.
- Credits are non-transferable and have no cash value.
- CANCELLATION & EARLY TERMINATION
- Early Termination: If the Member wishes to cancel prior to the end of the 6-month commitment, a fee equivalent to one month of membership dues will be charged, except in cases of relocation (verified by proof) or medical necessity (verified by a physician’s note).
- Notice: All cancellation requests must be submitted in writing to [info@velvetglowaesthetics.com].
- APPOINTMENTS & NO-SHOW POLICY
- Members must book appointments in advance.
- Cancellations must be made at least [24/48] hours in advance. Failure to cancel within this window may result in the forfeiture of that month’s treatment credit.
- MEMBERSHIP PAUSE Members may request a one-time "Membership Pause" of up to [30 days] due to medical leave or extended travel, provided the request is made in writing at least 15 days in advance.
